From the West: Head west on I-70 and take exit 147 to Eby Creek Road. Turn right on Eby Creek Road, then go straight through the roundabout and continue. Once you reach a second roundabout, take your first exit (right) onto US-6 W /Grand Ave and drive one mile. At the roundabout, take the 3rd exit onto Sylvan Lake Rd. Drive about .3 miles and Eagle Valley Care Center will be on your left.

From the East: Head East on I-70 HW and take exit 147 to Eby Creek Road. At the roundabout, take the 3rd exit onto southbound Eby Creek Rd, then go straight through the next two roundabouts. Once you reach the fourth roundabout, take your first exit (right) onto US-6 W /Grand Ave and drive one mile. At the roundabout, take the 3rd exit onto Sylvan Lake Rd. Drive about .3 miles and Eagle Valley Care Center will be on your left.
